CRITICAL CARE MEDICINE FELLOWSHIPThe Anesthesiology Critical Care Fellowship program is based in the Surgical and Cardio-thoracic intensive care unit at the Dallas VAMC and the Surgical/Trauma intensive care unit at Parkland Memorial Hospital.

During the first months of training, fellows will be working in these two units. Required rotations and elective rotations will be scheduled during clinical rotations in these ICU’s.

The experience and knowledge gained during the Critical Care Medicine fellowship should provide fellows with the expertise to manage critically ill patients in an intensive care unit. This management includes clinical assessment of the patient, determination of the use of a wide range of diagnostic procedures, interpretation of the collected data, and determination of appropriate therapy.

OVERVIEW
The fellowship program in Anesthesiology / Critical Care Medicine at UT Southwestern Medical Center in Dallas is designed to provide a complete 12 month training experience in all aspects of care of critically ill patients.

Broad familiarity and expertise in the multidisciplinary approach to Critical Care Medicine will be emphasized.

Four months of training takes place in the intensive care units at the Dallas Veterans Affairs Medical Center and four months takes place in the intensive care units at Parkland Memorial Hospital. In order to meet the ACGME requirement, one additional ICU rotation month can be completed at the Parkland Memorial Hospital, Dallas VA Medical Center or Children's Medical Center Dallas.  Direct faculty supervision is provided.

The remaining three months of training are electives of advanced clinical and clinical research based upon the fellow’s interests and abilities. Electives are also available in the Burn ICU at Parkland Hospital, Neurology/Neurosurgical ICU at Zale Lipshy university hospital as well as the pediatric ICU at Children’s Medical Center.  Didactics are emphasized with daily teaching rounds with attending intensivists from the departments of Anesthesiology, Pediatrics and Surgery. Fellows are strongly encouraged to be involved in clinical research.

Lectures, Journal Clubs, and morbidity and mortality conferences are regularly scheduled and fellows actively participate in these formal didactic sessions. At the completion of training, the fellow will have gained proficiency in the diagnosis and treatment of a wide range of illnesses and will be able to function in the role of consultant intensivist.

In addition to its emphasis on learning patient care, the critical care fellowship program is structured to provide opportunities in teaching, research, and administration.

In learning to be a consultant, the fellow must learn how to be a teacher. The Fellow will be asked to prepare formal lectures for Medical Student, Resident, and Fellow level conferences.

In the last several months of training, the fellow will be asked to act in the role of “Junior Attending” and will have increased responsibility in patient management decisions and teaching.

Research opportunities are available for the fellow to participate in ongoing research projects or to begin a new project. Education in hypothesis development, study design, ethics of performing studies on human subjects, Institutional Review Board processes, and project funding will be incorporated into the training. The fellow will have ample opportunity to review and publish the results of research projects.

The Fellow will gain experience in ICU administration through observation and increasing responsibility for a number of administrative issues. The fellow will gradually share responsibility for bed triage, training of ancillary personnel in critical care and ongoing review of intensive care unit outcome data. When training is completed, the fellow will be familiar with the administrative issues involved in the running of an Intensive Care Unit.

At the completion of the training program, the successful fellow will:
1. Demonstrate competence and professionalism all aspects of the management of the patient with critical illness
2. Provide evidence of proficiency in critical care procedures
3. Demonstrate the ability to function as part of a multi-disciplinary ICU team.

EVALUATION OF FACULTY BY THE FELLOW:
The fellow will be asked to complete an evaluation of the Critical Care Medicine faculty with whom he/she has worked.

EVALUATION OF ROTATION BY THE FELLOW:
The fellow will be asked to complete an evaluation of each rotation.

SUPERVISORY LINES OF RESPONSIBILITY:
While on the rotation, the fellow is immediately responsible to the Attending Physicians of record of that service. This applies irrespective of the department affiliation of the Attending Physician. If the Attending Physician is unavailable, the Medical Director of the ICU is the person of responsibility. The ICU Attending or Medical Director will report directly to the Fellowship Program Director any issues directly related to performance of the Critical Care Medicine Fellow. The Program Director will report directly to the Anesthesiology Department Chair / Core Residency Program Director.  While on elective or other non-ICU time, the Fellow will be immediately responsible to the Fellowship Program Director.

APPLICATION PROCESS:
Applicants must have successfully completed an ACGME accredited program in Anesthesiology or be in training and in good standing in their residency.

A completed application form and the following items should be submitted: 
• A recent photo
• A current curriculum vitae
• Personal statement
• A copy of your Dean’s Letter
• Original or notarized copy of your medical transcripts
• One letter of recommendation from your department chair in which you completed your residency training
• Two letters from attending physicians on the teaching faculty
• A copy of your USMLE or Comlex scores
• A copy of your In-Training scores and percentiles
• A copy of ECFMG certificate (if applicable)

Once your application is complete, it will be reviewed by the fellowship committee and you will be notified if an interview has been granted.  Please note that we require a formal, in-person interview.
